Mr Fowler, who would not be aged 56, was last seen by family at his caravan home on Cabbage Tree Road at Williamtown on July 2 last year.
He had recently been diagnosed with high-level dementia and his sister Kathie Case and her husband, Geoff, regularly visited his property to check on him.
After failing to hear from him for four days, the Cases drove to Williamtown to find a light on but nobody home.
The sliding door was open, but his two cars were in the driveway and his tobacco was inside, along with his wallet, phone and medication.
“It was all the things that he never would have walked out without,’’ Ms Case said.
“We just don’t know what has happened to him.’’
The day after the Cases visited Mr Fowler, CCTV placed him at Williamtown McDonald's about noon, with footage from a police highway patrol car showing him sitting on the side of Nelson Bay Road, about 200 metres from the fast food restaurant, near the 100 km/h speed sign.
And then he vanished.
A massive land search over several days across Williamtown farming land failed to find any trace of Mr Fowler.
Police said they have continued to search the area on a regular basis, and ongoing inquiries have revealed Mr Fowler has not accessed his bank accounts since his disappearance.
Police and family hold serious concerns for Mr Fowler’s welfare due to his dementia, and are keen to hear from anyone who may have information in relation to his whereabouts.
Ms Case made the latest public appeal as police promote a series of cases as part of national missing persons week.
Mr Fowler is described as being of Caucasian appearance, about 172cm tall, of a thin build, with brown hair and blue/grey eyes.
Anyone with information about the whereabouts of Mr Fowler is urged to call Crime Stoppers on 1800 333 000 or use the Crime Stoppers online reporting page.
