When it comes to taking control of your health and wellbeing it can sometimes feel confusing as you navigate an often complex health system.

With so many services available how do you access the best options for you and those you care for?

Hunter Primary Care is a local organisation on a mission to alleviate the confusion. It's assisting Hunter residents achieve their health goals through a service delivery focused on helping individuals at any point along their healthcare journey.

Brenda Ryan, Chief Executive Officer of Hunter Primary Care, said many people in the community are not accessing the health and wellbeing services they need and that are available to them.

"Many people are unsure of how to find and manage all the services and supports that can give them the skills, choice and control to manage their wellbeing, both physical and emotional," Ms Ryan said.

"Not every healthcare journey is the same and complexities can arise, especially when coordination of multiple service providers is needed."

For years Hunter Primary Care's Care Coordination team has worked with local GPs and members of the community to help manage people's complex health issues including diabetes, cardiovascular disease, and long-term renal disease.

"Success through this service and our local Partners in Recovery service has allowed Hunter Primary Care to successfully enter the National Disability Insurance Scheme market offering Support Coordination services," Ms Ryan said.

With a history spanning 27 years, Hunter Primary Care continues to build upon its network of trusted health professionals to deliver quality primary health, mental health, National Disability Insurance Scheme (NDIS) Support Coordination and after-hours care to the Hunter region.

Through collaboration with leading health organisations, Hunter Primary Care delivers a range of services to help people connect and manage the services they need.

Ms Ryan said Hunter Primary Care's aim was to support people to lead a healthy life by connecting them with the most experienced professionals to advise and support them.

"We strive to not only be a provider but a partner to the community on their healthcare journey," Ms Ryan said.

"We're passionate about helping people achieve their goals and reach their potential so they can live the life they want."

Hunter Primary Care's wide range of services covers four categories - Aboriginal Health and Wellbeing, Disability and Wellbeing, General Health and Wellbeing, and Mental Health and Wellbeing.

Some of its successful programs include:

Yudhilidin - A recovery-oriented care coordination service assisting Aboriginal and Torres Strait Islander people with complex mental health needs access support from a GP and other health services to improve health, social and emotional wellbeing.

headspace Newcastle - Information and support on a wide range of issues for young people aged 12 to 25 staffed by youth-friendly professionals who can talk about problems including mental health issues, drug and alcohol issues, school stress, and family and relationships.

The Wayback Support Service - Non clinical care and support to people for up to three months following a suicide attempt.

Psychology Services - Providing responsive and effective psychological intervention to people with mild to moderate mental health and drug use difficulties.

NDIS Services - Comprehensive support and coordination from a team that works with NDIS participants with everything from understanding their plan to connecting them with services to achieve their goals and get the most value out of the NDIS.

General Health and Wellbeing Services - Providing occupational therapy, dietitian support, exercise physiology, management of long-term illness and after hours medical advice when your GP is closed.
