Police said the incident occurred at the Government Road business about 2.30pm after a verbal argument broke out between a couple - a 26-year-old man and a 30-year-old woman - and a 25-year-old female customer who allegedly left the premises to film them as they left.
Several customers, including children, were in the café at the time.
When the couple re-appeared inside the cafe, police said, the altercation became physical inside.
Another female customer intervened and tried to separate the parties.
The man and woman left the premises a second time. No reports of injuries have been received.
Port Stephens-Hunter duty officer Acting Inspector Kristin Marshall said police were concerned about commentary related to the incident that had appeared on social media.
Inspector Marshall said no-one employed by the cafe was involved in the incident.
Police are appealing for any witnesses or anyone with information about the incident to contact Raymond Terrace detectives or Crime Stoppers on 1800 333 000.
