Police said the man, 37, was one of three people who left a golf club at Shelly Beach in a buggy around 11.40pm on Friday.
The buggy turned on to Grandview Street before allegedly turning sharply to avoid an unknown vehicle, leading to the man falling from the cart and hitting his head on the road.
He was taken to John Hunter Hospital in a medically induced coma with a possible fractured skull and a bleed on the brain.
A Hunter New England Health spokesperson said he was in a serious condition on Sunday.
The buggy driver, an unknown woman, left before officers from Tuggerah Lakes Police District arrived.
They spoke to another man, 52, who was travelling in the buggy.
Police have appealed for the driver, plus any witnesses or anyone with dashcam footage, to contact police.
