Emergency services were called to Catherine Hill Bay at the intersection of Cams Wharf Road and Flowers Drive just before 8am Tuesday.
"It's understood a Honda sedan travelling north and a BMW sedan travelling south have collided head-on," a police statement said.
"A female passenger of the Honda was treated at the scene by NSW Ambulance paramedics but died a short time later."
The BMW contained four elderly people, three females and one male, who were all treated by paramedics at the scene. Three were taken to John Hunter Hospital, one in a serious condition suffering chest injuries, the other two treated for minor cuts and bruises according to a spokesman from NSW Ambulance.
The Pacific Highway was closed for most of the morning. It reopened to traffic just after midday, with earlier diversions lifted and buses returning to their normal route.
The crash site has been cleared and traffic is quickly returning to normal.
A report will be prepared for the information of the coroner.
