Police said emergency services were called to Thomas Mitchell Drive about 6.30pm on Tuesday after reports a Hyundai Getz and a Nissan Pulsar collided.
The driver of the Hyundai, a 28-year-old man, died at the scene. He was the only occupant of that vehicle.
The Nissan's driver, 20, was freed from the wreckage by firefighters. She was flown to the John Hunter Hospital in a serious condition while her first passenger, a 10-year-old girl who had been travelling in the back seat, was also taken to hospital with suspected fractures.
A seven-week-old boy in the same vehicle escaped injury but went to hospital for precautionary checks.
Hunter Valley Police District officers established a crime scene, which is being forensically examined. An investigation is underway into the circumstances of the crash and a report will be prepared for the coroner.
Investigators are appealing for anyone with dash-cam vision or information about the incident to call Crime Stoppers on 1800 333 000.
The Muswellbrook crash occurred just a day after a truck driver died in a single-vehicle accident on the Golden Highway at Jerrys Plains.
