Emergency services were called to Main Road about 5.15pm to reports of a multi-vehicle collision.
Six ambulance crews, police, NSW Fire and Rescue, the Westpac Rescue Helicopter and the Cessnock District Rescue Squad were sent to the scene.
On arrival crews found three vehicles had been involved in the crash, with one man trapped inside his car.
"The vehicle was stabilised before our volunteers began the task of freeing the man," a statement from the rescue squad said.
"With relief cuts made, hydraulic cutters and spreaders were then used to remove the drivers door which gave the medical team a greater viewpoint of the man's legs.
"Whilst they assessed the situation our team had our hydraulic rams ready in preparation to push the dash forward so he could be freed."
The man, believed to be in his 30s, suffered serious leg injuries in the crash and was taken to John Hunter Hospital in a stable condition.
A second person also suffered minor injuries in the crash and was also taken by ambulance to John Hunter Hospital for further treatment.
