Elizabeth Grist, Hunter New England Health's COVID-19 response lead, confirmed a person who attended the hub on Thursday, August 5, between 11 and 11.45am, tested positive to the virus five days later on August 10.
"The centre has been thoroughly cleaned and NSW Health is contacting casual and close contacts," Ms Grist said.
"The vaccination centre remains open and people are encouraged to continue to come forward for their vaccination appointment if they are well.
"You must not attend if you have any symptoms however mild. If you have been asked to isolate because you are a close contact of a confirmed case, you must not attend the vaccination centre.
"If you are a casual contact of a confirmed case, you must have a negative COVID-19 swab result before attending the centre for a vaccination."
Ms Grist said all visitors and staff at the vaccination centre are required to wear masks at all times, and physical distancing is observed inside the centre.
Planet Fitness Lambton, on Turton Road, has also been listed as an exposure site.
Anyone who attended on Tuesday, August 3, from 3.05 to 5pm, is considered a close contact and must get tested and isolate for 14 days since they were there, regardless of the result.
