Paramedics and the Westpac rescue helicopter were sent to Swan Bay Road about 9.30am after reports of a truck had rolled west of Karuah.
It was reported from the scene that a man was trapped with serious injuries and a woman was outside the truck with minor injuries.
Crews arrived and began work to free the 28-year-old man while treating his wounds. Emergency services eventually freed the driver, who had suffered pelvis and arm injuries.
Paramedics and the helicopter's critical care medical team stabilised the man before he was flown to the John Hunter Hospital.
A Westpac Rescue Helicopter Service spokesman said the patient was transported in a serious but stable condition.
Medical crews also assessed the female passenger, 25. She was driven to hospital by ambulance with minor injuries.
