A 79-year-old woman driver died when the car she was driving failed to give way and hit a Nissan ute at the Fogarty's Gap Road intersection of the Calder Highway.
The female driver of the Corolla died at the scene and is yet to be formally identified.
An 83-year-old woman travelling in the Corolla was air-lifted to The Alfred hospital in a critical but stable condition.
The woman was trapped for some time before SES crews and paramedics freed her.
The four people travelling in the ute received only seat belt injuries.
"The red vehicle was travelling east on Harmony Way to come across the freeway and has failed to give way at that intersection," Acting Senior Sergeant Della Nihill said.
Acting Senior Sergeant Nihill said the intersection was known as a dangerous stretch of road.
"It has been in the past but not for some time.
"It is just unfortunate that we now have another fatality."
Acting Senior Sergeant Nihill said the speed limit at the intersection would be assessed.
"That will be part of the assessment as a result of the fatality and be taken on by Vic Roads," she said.
"We are urging motorists to slow down, take their time and look at the conditions around them."
The fatality follows the death of an elderly Bendigo woman who died after her car veered off the road and hit a tree in Archdale near Maryborough on Sunday.
Investigators believe a Toyota Corolla was travelling along Archdale-Dunolly Road about 4.15pm when the driver lost control of the car.
The 84-year-old woman and only occupant of the vehicle died at the scene.
