Up to 750 individuals from an estimated 50 teams and six grades are being asked to choose a preference from three different options, all scheduled to start on July 18.
Two proposals feature 14 rounds, but one with a straight-out decider on October 31 and the other with three weeks of finals culminating on November 14.
The third motion is a shorter campaign with 11 rounds, finishing on September 29, and no play-offs. Winners would be determined by the minor premiership.
Surveys were sent out last month via email, social media and the NDWHA website.
"Hopefully everyone votes," NDWHA vice-president Sharon Williams said.
"Once we have that answer, we will then hold a Zoom meeting with clubs to re-evaluate what clubs and teams will participate."
The outbreak of COVID-19 saw NDWHA postpone all competitions on March 16.
In terms of this year's NDWHA premier league, Erina-Avoca Beach had already pulled out reducing that division to eight teams.
Gosford and The Entrance are the remaining Central Coast entities. Two-time defending champions Oxfords, last year's grand finalists Souths, Tigers, Central, Regals and University make up the Newcastle sides.
Meanwhile, construction work on Newcastle International Hockey Centre redevelopments were set to begin this week following the recent installation of new lights.
