Police and emergency services were called to the Pacific Highway near Viney Creek Road West about 1.40pm after reports two cars and a motorcycle had collided.
"Police have been told an Isuzu D-Max utility travelling southbound collided with a motorcycle and a Toyota station wagon, both travelling northbound," NSW Police said in a statement.
The male motorcyclist died at the scene.
The Toyota's female driver also died at the scene after she was trapped by confinement within the wreckage.
Police said the man and woman were yet to be formally identified.
Paramedics treated a woman who had been riding in the Toyota as a passenger, taking her to the John Hunter Hospital.
The Isuzu's male driver suffered minor injuries.
Police said he was taken to the John Hunter Hospital for mandatory blood and urine testing.
Port Stephens-Hunter police district officers established a crime scene.
Crash investigators will examine the circumstances of the crash, which closed northbound lanes of the highway. They were expected to be closed for a significant amount of time, police said, with contraflow in place in one of the two southbound lanes.
Drivers have been urged to expect delays and drive to the conditions.
Police said there was "a large amount of water across the roads from wet weather today".
A report will be prepared for the coroner.
