POLICE say they have serious concerns for the welfare of a Lake Macquarie man living with dementia last seen on Monday evening.

Keith Metcalfe, 71, was last seen at a Booragul facility on Toronto Road about 6.30pm.
Lake Macquarie police were informed a short time later and began efforts to find Mr Metcalfe.
He is described as being of Caucasian appearance, about 180cm tall, of medium build, with a bald head and grey goatee.
He was last seen wearing a striped white top with a blue collar and brown pants.
